TCM – Chinese Medicine and Diagnosis Consults

| TCM-001 Chinese Medicine and Diagnosis INITIAL Consult |
| Appointments with our licensed staff practitioners of traditional Chinese medicine include a full review of your medical, fertility and health histories. Upon receiving your intake forms and photographs of your tongue and face (guidelines are provided for accomplishing this), your practitioner will study them and prepare for your scheduled appointment. Approximately one-week after your appointment, we will provide you with a written treatment plan that includes your Eastern Medicine diagnosis and explanation of what it means, our recommended treatment objectives, a custom herbal prescription and advice regarding nutritional supplements to address your diagnosis. Your treatment plan may also suggest a limited number of dietary and lifestyle modifications to support you in addressing your imbalances. You fill your herbal prescription through The Fertile Soul store, a recommended pharmacy for pre-made teas and raw herbs, or the Chinese herbal pharmacy of your choice.
This consult program includes:
- A review by one of our licensed acupuncturists of your full health and medical history
- An initial 60-minute phone consultation
- A Chinese medicine diagnosis and detailed written custom treatment plan
- A prescription for an herbal formula developed specifically to address your individual diagnosis

| TCM-002 Chinese Medicine and Diagnosis FOLLOW-UP Consult |
| This program is for clients who have had a TCM consultation by phone or attended a retreat within the past three months. It is designed to help you monitor improvements in your health resulting from your fertility therapies and modify your herbal formula accordingly, as well as continue to adopt new practices to enhance your fertility. It includes one 30-minute phone consultation in which you will discuss your changes in symptoms with your TCM practitioner, who will then develop and provide you with a new herbal prescription (if indicated) and other recommendations as appropriate.
TCM Follow-up Consult $100
PRIOR REQUIREMENTS: Must have one of the following within past three (3) months: Initial and/or Follow-up TCM Consults (includes Combination Packages) or attended a Fertile Soul Retreat

| TCM-003 Chinese Medicine and Diagnosis HOME PRACTITIONER consult |
| This appointment with our licensed staff practitioners of traditional Chinese medicine includes a one-on-one review of your medical, fertility and health histories with your home practitioner.
This consult program includes:
- A review by one of our licensed acupuncturists of your full health and medical history with your home practitioner
- A 15-minute phone consultation
TCM Practitioner-to-Practitioner Consult $50
